Fixed teeth in 24 hours — All-on-4 and All-on-6

A complete oral rehabilitation for patients who have lost their teeth, or whose remaining teeth are beyond saving. Where the case allows, the fixed temporary bridge goes in on the day of surgery.

What All-on-4 and All-on-6 actually are

Four or six implants are placed at planned angles in the upper or lower jaw, and a single fixed bridge is carried on them. The choice between four and six is not a preference or an upsell: it follows from your bone, from how stable the implants are once placed, and from what the arch has to do when you chew.

Who the treatment is for

  • People who have lost all, or nearly all, of the teeth on an arch;
  • people whose remaining teeth cannot be saved and need extracting;
  • people wearing a removable denture that will not stay put;
  • people who want something fixed, stable and comfortable.

Moderate bone loss does not rule you out — the angled placement is precisely what makes those cases treatable. If you have already been told there is not enough bone, that is a reason to send us the scan rather than a reason to give up: send it for a second opinion and we will tell you plainly what is and is not possible.

The stages of treatment

  1. Consultation and CBCT imaging

    A CBCT scan gives a detailed picture of the bone: how much there is, what quality it is, where the nerves run, and therefore where each implant can safely go. It is quick, painless and low-dose. If you are travelling to us, send a recent scan beforehand and we will review it before you book anything.

  2. Surgery — extractions and placing the implants

    Under local anaesthetic, any teeth beyond saving are removed and the implants are placed to plan. Allow roughly two hours per arch, depending on how complex the case is.

  3. Fitting the temporary bridge — within 24 hours, where the case allows

    After surgery a fixed temporary bridge is fitted, made from a digital impression taken with an intraoral scanner. It gives you working teeth and an acceptable smile straight away, throughout healing.

  4. Osseointegration

    Then comes osseointegration, the months in which the implants fuse with the bone. It usually takes four to six months and it is what makes the result stable in the long run. There is no shortening it.

  5. The definitive bridge — fitted and finished

    The definitive work normally takes two or three appointments (digital impression, try-in and final adjustment), so that the fit, the bite and the look are all right rather than merely acceptable.

When fixed teeth in 24 hours are not possible: if the implants do not achieve enough primary stability, or the bone conditions are not favourable, the fixed temporary is postponed and treatment proceeds in stages instead. That decision is made in theatre, in your interest, and it is the reason the result lasts.

Afterwards

The surgery is carried out under local anaesthetic, so the procedure itself is not painful. Afterwards there can be some soreness or discomfort, controlled effectively with medication — an antibiotic, painkillers and an anti-inflammatory, as prescribed.

About three to five days for the first stage — consultation, surgery and fitting the temporary bridge — so you fly home with fixed teeth. The definitive work needs a second visit after osseointegration, usually three to six months later, with a stay of two to four days. We plan both around your travel.

Yes, and you should. Send photographs and a recent CBCT scan or panoramic x-ray by WhatsApp or e-mail. Dr. Man will review them and tell you what is realistic, how many implants your case is likely to need and roughly what it will cost — before you commit to anything.

Normally the discomfort is minor. You may feel some tenderness or a sense of pressure in the gum, which is temporary and settles quickly with the medication we prescribe. The temporary bridge is shaped specifically so that it does not press on healing gum. Most patients are back to normal activity the next day.

No. It is carried out under local anaesthetic and you will not feel pain during the procedure. Afterwards there can be mild discomfort or a feeling of pressure, which is normal, temporary, and well controlled by the medication we prescribe.

In most cases, yes. We go through your medical history carefully beforehand and, where it is needed, we liaise with your cardiologist. The surgery is done under local anaesthetic, which keeps the physiological load low.

In many cases, yes, provided the diabetes is well controlled — ideally an HbA1c below 7–7.5%. We use PRF membranes, which speed up healing in both soft tissue and bone. Following the post-operative instructions and keeping to the review appointments matters more here than in most cases.

No. Every patient needs a detailed clinical and imaging assessment. Bone volume, general health and habits such as smoking all bear on the decision. Where immediate fixed teeth are not indicated, we say so and recommend the alternative that suits the case — that honesty is the point of the assessment.

Medical ethics do not allow a dental outcome to be guaranteed, and any clinic that promises one is telling you something it cannot deliver.
